Lexington Place Nursing & Rehabilitation Center welcomes payment from the following sources: Medicare, Medicaid, private pay, insurance and Hospice. Now accepting payments from Visa, MasterCard and Discover.
Medicare provides coverage for skilled nursing and rehab services following a qualifying hospital stay (three night in-patient stay). Medicare will provide 100 days of coverage. Twenty days are covered fully by Medicare with no co-insurance cost. Day 21-100 are covered partly by Medicare with a co-insurance of $152 per day. All or part of the co-insurance rate is usually covered by Medicare supplement plans. Medicare covers the cost of 24-hour care, personal care assistance, rehabilitation, skilled nursing services, medication, medication administration, meals and dining, housekeeping and laundry, daily activities and medical social services.
Nursing Home Medicaid covers the cost of long- or short-term care. Eligibility is determined by Department of Aged and Disabled Services (DADS). Submission of the application must be preceded by a 30-day consecutive stay in a long-term care facility. Patient share costs are determined by Medicaid guidelines. Medicaid covers semiprivate accommodations, 24-hour nursing care, personal care assistance, medication and medication administration, meals and dining, housekeeping and laundry, daily activities and medical social services.
Lexington Place offers private and semiprivate accommodations. Private rooms are available at the rate of $150 per day. Semiprivate rooms are available at the rate of $125 per day. This cost includes 24-hour nursing care, personal care assistance, medication administration, meals and dining, housekeeping and laundry, daily activities, and medical social services.
Private insurance will usually pay a percentage of reasonable and necessary services. Deductible and coinsurance amounts vary depending on the plan type. Precertification is required.
